Solutions provides effective tools to improve health care practices and practice administration, addressing key concerns in public health care policy today.  Its standard set of metrics can be compared across departments in a medical group, among unaffiliated medical groups, and across geographies, providing solid foundations for discussions of policy alternatives.  By making the software available to safety-net clinics for free, their data can be included in aggregations and norms and their quality improvement efforts can be supported as well as major groups' needs are.  By identifying the key data subset needed for good practice analytics, the resulting data volume is much more manageable and exchangeable.  By focusing on expert metric sets and comprehensible presentations, Solutions delivers in useable ways the profession's best thinking about quality improvement.

  • Solutions Design

    Solutions creates optimized data bases of essential information and metrics related to medical practices and presents selected information to clinicians and administrators to support decision making and improvements. It takes data from existing sources, which is usually 'dirty' and requires a lot of human interpretation. It filters out most of it as unnecessary, cleans it up, normalizes it, applies expertise, and presents the results with easy-to-understand graphics.

    Tomás's research made two major contributions to medical quality improvement which are at the core of Solutions.

    1. He identified the small sub-set of total EMR data that is actually useful in practice management.

    2. He identified the key metrics needed for practice management decisions.

  • Additional Modules

    The advanced architecture of Solutions permits the reliable and easy addition of other modules and technologies, of which there are already several. One is a treatment management module that can watch for preventive action opportunities, abnormal test results, and patient compliance with chronic care instructions. Another is the ability to connect to traditional communication tools such as mail-merge for email, printers for postal mail, and automated phone calling systems to communicate reminders and other information. A third comprises tools to support health management of groups of patients with preventive and followup actions.

Technology 

Solutions has 3 core modules:

1. The Harvesting Engine gathers the data from available electronic sources, then filters and cleans it. The harvesting is done by data extractors that are customized for each data source. Currently Solutions has general extractors for Epic and Centricity and can build extractors for other sources. In situations where there are no electronic sources for needed data, Solutions can use custom-built web forms to capture the data manually.

2. The Calculation Engine applies expertise to the clean data to create useful metrics. It comes pre-loaded with more than 150 key improvement metrics. Users can create new metrics by using tools that HMS offers. Some examples of current metrics are:

• productivity measures such as RVU per provider FTE and variable cost per RVU

• operational measures such as visit counts, patient demographic distributions, and panel size• clinical measures for rating diabetic care such as the glyco-hemoglobin control and the frequency of glyco-hemoglobin testing

• clinical measures for rating diabetic care such as the glyco-hemoglobin control and the frequency of glyco-hemoglobin testing

3. The Presentation Engine delivers the metrics in easy-to-use, customizable forms, either in familiar Excel Workbooks or in web pages with a similar appearance. It allows users to create new views and to share them with colleagues.
Solutions currently runs on Microsoft's SQL Server, but is being ported to open source PostgreSQL.
