Solutions creates optimized data bases of essential information and metrics related to medical practices and presents selected information to clinicians and administrators to support decision making and improvements. It takes data from existing sources, which is usually 'dirty' and requires a lot of human interpretation. It filters out most of it as unnecessary, cleans it up, normalizes it, applies expertise, and presents the results with easy-to-understand graphics.
Tomás's research made two major contributions to medical quality improvement which are at the core of Solutions.
1. He identified the small sub-set of total EMR data that is actually useful in practice management.
2. He identified the key metrics needed for practice management decisions.
Solutions has 3 core modules:
1. The Harvesting Engine gathers the data from available electronic sources, then filters and cleans it. The harvesting is done by data extractors that are customized for each data source. Currently Solutions has general extractors for Epic and Centricity and can build extractors for other sources. In situations where there are no electronic sources for needed data, Solutions can use custom-built web forms to capture the data manually.
2. The Calculation Engine applies expertise to the clean data to create useful metrics. It comes pre-loaded with more than 150 key improvement metrics. Users can create new metrics by using tools that HMS offers. Some examples of current metrics are:
• productivity measures such as RVU per provider FTE and variable cost per RVU
• operational measures such as visit counts, patient demographic distributions, and panel size• clinical measures for rating diabetic care such as the glyco-hemoglobin control and the frequency of glyco-hemoglobin testing

3. The Presentation Engine delivers the metrics in easy-to-use, customizable forms, either in familiar Excel Workbooks or in web pages with a similar appearance. It allows users to create new views and to share them with colleagues.
Solutions currently runs on Microsoft's SQL Server, but is being ported to open source PostgreSQL.
